Our complete application is below.  Our references (including our vet) were never called, and I never received an email to get our results.  I called to see when I might be able to schedule a meeting with our prospective new family members, and was told verbally by the secretary that our application was denied.  She couldn't tell me why, and she couldn't connect me to anyone who could.  She said I could send a follow-up email to the Board, but that they only check that email about once each month.  She also said that ALL decisions were FINAL, and that they had the right to deny us without answering questions.

Have you ever had to give up a pet? Yes
If yes, please give details why & what happened to each pet/where did each pet go...If No, put N/A:
I found a new home for Max when he was a year old. In February of 2006, my husband died of a brain aneurysm. My children were 11 and 4 at the time, and I thought I should get a large dog as protection. In retrospect, I wasn't in the right frame of mind to make long-term decisions. Max was WAY too big for our house and small yard (90 pounds at 6 months, over 120 pounds on his first birthday), and he was bred to be a working dog (flock guardian). We weren't a good match for him. I located a family that had a goat farm (needed a flock guardian), and whose young son wanted
to have a dog in the 4-H program. After meeting with the family several times, I gave Max to them.

Why are you looking for a new pet & what characteristics are you looking for?
We weren't looking for another pet, but we saw Oscar's picture on Facebook, and something clicked. I went to your website, and read about him and his sisters, and found out that they are part Belgian Shepherd. I've ALWAYS wanted one, but I can't see buying a purebred dog when there are so many puppies that need a home. That's why this seems so perfect to me!
Why are you interested in this particular pet?
See above. I think a Belgian Shepherd/retriever cross just might be the perfect dog (except for my Petunia Sugar-Biscuits - she's perfect too!)
Why do you feel you are ready for a new pet & why would you make a great home for this pet?
We're very loving pet owners, and our four-legged family members seem very happy. We take our responsibility as pet owners very seriously. We don't feel the need for another pet, but if the right opportunity presented itself, we would be happy to bring another into our family. We think perhaps Oscar, Sophie or Bridget might just be our next family member. I think we make a great home for our pets for a number of reasons. I work from home (with some occasional travel), so our pets aren't left alone all day every day. We live in a rural area, and take many walks. My husband also like to jog. We include our dog in many family activities, including our wedding last year.
Where will this pet be kept during the day or evening while you are away from home? Please provide details (exactly where - indoors, outdoors, confined to area, crated,etc..) Define the area.:
I work from home, so I'm here all day every day (unless I'm traveling - which happens occasionally). Our pets have complete access to the inside our house at all times (even when we go run errands). When I have to travel for work
(several times per year), the animals are alone in the house from the time my son  leaves for school until he arrives home in the afternoon (approximately 8 hours). They sleep with us on our beds.
How long will the pet be alone, with no human companionship, each day on average?
0 hours
Where will your new pet sleep?
in our room (floor or bed)
Where will your new pet live? Please provide thorough details.: Indoors
How long will your pet be outdoors or in an out building (not in your main home) on average each day/night?
Oscar/Sophie/Bridget will live indoors, and have the full run of the house. Since I work from home, I'll be able to take him/her out frequently during the house-training process and afterwards. We'll take daily walks before and/or after work.
If yard is not fenced, how do you plan to keep your new pet confined to your property (tie-out, dog run, dog pen, only on leash, etc..)? Please provide details of the 'system' you use to ensure your pets' safety.:
Obviously, we would not use the fenced pool area as a fenced yard. Last year,  our older son lived with us, along with his beagle. Occasionally, when he felt that she needed more outside time (he worked MANY hours), he would let her run on a zip-line attached to our house. When he moved out this past summer, he took the dog and zip-line with him. However, if you feel it's warranted, we could always put another up.
Who will be the primary caregiver(s) to this pet? (Name/Age) Please provide details of the primary caregiver's
daily schedule (work hours, hours away from home, etc...)
:
1. Tara - age 47. Works from home
2. Peyton - age 11. Goes to school from 7:30am - 3:30pm
What do you think it will cost per year to care for this pet? Please list expenses:
Food - $350
Vet - $250
How often do you think your pet will need to see a veterinarian? Please detail reasons:
Sugar-Biscuits sees her vet yearly for vaccinations and check-up, and as needed if she's ill. I would imagine that would be the case for a new dog too.
If your pet has or develops behavior issues (urinating, chewing, scratching, digging, excessive barking...), how will you handle?
I would check with our vet and research the issue online. Since I have a lot of time at home, I could work with him/her to resolve the issue.
What amount of time do you consider reasonable for your new pet
to adjust & adapt to his/her new life in your home?

4 weeks?
If something life changing should happen to you (other than death or
needing to move into a nursing home/assisted living facility) which makes the pet an obstacle or inconvenience (i.e.-move, divorce, kids leave home, lose job, having children, etc…) what would you do? Please elaborate on individual circumstances you feel may affect your ability to care for or keep your pet.
:
I guess I would contact Joyful Rescues first for advice.
What circumstances might lead you to want to return your pet to us?
If the dog presented a danger to my family (i.e. aggressive behavior), I would contact you.
Please tell us more about yourself, your family, your home, your neighborhood, your lifestyle, etc......... Your final 'sales pitch' to us that you are the 'right' home for this pet.:
I hope you have a pretty clear picture of our family at this point. We are a loving, fun, responsible family. If you think Oscar/Sophie/Bridget would benefit from joining us, I hope you'll give him/her that chance. (And Petunia Sugar-Biscuits says she misses having a doggie friend around!) Thanks!
